تأثیر مراقبت از دهان در پیشگیری از پنومونی ناشی از تهویه مکانیکی: یک مطالعه مروری روایتی

مقدمه: نقش پرستاران برای پیشگیری از پنومونی ناشی از تهویه مکانیکی امری ضروری است لذا این مقاله مروری با هدف تعیین تأثیر مراقبت از دهان در پیشگیری از پنومونی ناشی از تهویه مکانیکی در بیماران بستری در بخش‌های مراقبت‌های ویژه انجام شد.
روش کار: از مطالعات کارآزمایی بالینی منتشر شده ازسال 2005 تا 2015 میلادی در زمینه نقش مراقبت از دهان در بروز پنومونی ناشی از ونتیلاتور استفاده شد. متن کامل مقالات از طریق جستجوی در پایگاه‌های قابل دسترس مانند SID، Pubmed، Iran Medex، Elsevier، Springer، Magiran،Google scholar بدست آمد. همچنین از کلید واژه‌های "oral care"، "oral hygiene"، mouth care""،"oral hygiene practices"،"mouth hygiene"، "critical care"،"intubated intensive care"،"intubated mechanically ventilated"، "ventilator-associated pneumonia" و معادل فارسی در عنوان و یا چکیده جستجو شده و مورد بررسی قرار گرفتند. در این مطالعه از پروتکل spirit جهت ارزیابی مقالات استفاده شد.
یافته‌ها: ابتدا تعداد 1432 مقاله به دست آمد که پس از وارد کردن معیارهای ورود به مطالعه در نهایت تعداد 32 مقاله انتخاب شدند. مراقبت از دهان با محلول کلرهگزیدین و آنتی سپتیک بطور چشمگیری از پنومونی ناشی از ونتیلاتور پیشگیری می‌کند. مسواک زدن در کاهش پلاک‌های دندانی که محل تجمع میکروارگانیسم‌های ایجاد کننده پنومونی ناشی از ونتیلاتور می‌باشد تأثیر چشمگیری دارد. اجرای برنامه مداخله مبتنی بر شواهد کیفیت مراقبت از دهان را بوسیله پرستاران بهبود می‌بخشد و آگاهی در مورد راههای ایجاد پنومونی ناشی از ونتیلاتور می‌تواند باعث افزایش اهمیت دهی به مراقبت‌های دهانی گردد.
نتیجه گیری: مراقبت از دهان نقش مهمی در پیشگیری از پنومونی ناشی از ونتیلاتور دارد و نیاز است دانش پرستاران در مورد پنومونی ناشی از ونتیلاتور و مراقبت از دهان ارتقا داده شود.
