Introduction: Treatment adherence plays an important role in improving the physical condition of chronic patients and reducing disease recurrence. This study aimed to compare the effectiveness of acceptance and commitment therapy (ACT) and logotherapy based on Rumi's thoughts (LBRT) on treatment adherence in patients undergoing hemodialysis. Methods: The present study was a quasi-experimental study with a pre-test-post-test design and a control group with a follow-up period of 45 days. The statistical population of this study included all hemodialysis patients in Sari in 2024. 45 people were selected using the convenience sampling method and randomly assigned to three groups: experiment 1 (15 people), experiment 2 (15 people), and control (15 people). The Morisky Medication Adherence Scale was used to collect data. ACT and LBRT were implemented in groups for 8 sessions for the intervention groups. Data analysis was performed using repeated measures analysis of variance. Results: The results of the pairwise comparison between the groups indicated that there was a significant difference between the intervention groups of ACT (P=0.01) and LBRT (P=0.01) compared to the control group until the follow-up stage; in addition, the results of the pairwise scale between the two treatment groups of LBRT and ACT were not significant (P<0.05). Conclusion: In general, it can be concluded that both interventions of ACT and LBRT had the same effect on treatment adherence of patients undergoing hemodialysis over time.
