fa بررسی تجربیات دانشجویان پرستاری از خطاهای بالینی و چگونگی تعامل با آن پرستاری nursing مدیریت پرستاری Nursing Management and Administration ه: خطاهای بالینی تهدید جدی در امنیت بیمار هستند. مراکز آموزشی درمانی به دلیل برخورداری از دانشجویانی که در حال یادگیری‌اند، به شدت در معرض بروز طیفی از خطاهای پزشکی می‌باشند. هدف از این مطالعه بررسی تجربیات دانشجویان پرستاری از خطاهای بالینی و چگونگی تعامل با آن بوده است. روش: در این مطالعه که با رویکرد کیفی در سال 1389 انجام شده است ابزار جمع‌آوری داده‌ها مصاحبه‌های نیمه ساختار، متعامل و عمیق بوده است. بر اساس نمونه‌گیری مبتنی بر هدف با 30 دانشجوی دختر و پسر پرستاری از ترم‌های مختلف تحصیلی مصاحبه به عمل آمد. مصاحبه ضبط، دست‌نویس و داده‌ها مطابق روش تحلیل محتوای «مورس» آنالیز شد. یافته‌ها: دانشجویان مفهوم خطا در بالین را مترادف با به خطر افتادن جان بیمار و صدمه به بیمار می‌دانستند. عمده خطاهای رخ داده «دارویی و رویه‌های مراقبتی درمانی» و تجربه مقابله‌ای کارآموزان در برابر خطاهای مراقبتی «پنهان کردن» بوده است. آشکار شدن خطای مراقبتی با عصبانیت کادر درمانی مراقبتی، همراهان بیمار و مربی، ترس و عذاب وجدان فراگیران می‌شده است. شرکت‌کنندگان در مطالعه «عدم درخواست برای راهنمایی، آگاه نبودن، نظارت نداشتن مربی» را از عوامل مرتبط با بروز خطاهای بالینی به هنگام کارآموزی و یا کارورزی می‌دانستند. بیشترین خطا در زمانی بوده است که به کارآموز دستورات شفاهی داده می‌شد. نتیجه‌گیری: تجربیات دانشجویان نشان داد که خطا در بالین اجتناب‌ناپذیر است و شیوه مقابله‌ای آن‌ها پنهان کردن می‌باشد. ضروری است دانشجویان شیوه‌های اخلاقی و قانونی مدیریت خطای بالینی را بیاموزند. ضرورت وجود برنامه‌های گزارش‌دهی خطاهای بالینی جهت برنامه‌ریزی به حداقل رساندن آن‌ها بسیار مهم می‌نماید. Introduction: Clinical errors are considered as serious treat for patient safety. Since the students are in training process, therefore the patients in teaching hospitals are faced with variety of medical errors. The aim of this study was to explore clinical errors of nursing students and dealing with them. Methods: In this qualitative study, purposeful sampling semi-structured interviews were performed with 30 male and female nursing students in 2010. The interviews were transcribed line by line and were analyzed according to Morse's content analysis approach. Results: Clinical error was explained as "treat for patient's life or damage". The most common error was medical and procedural errors. "Hiding" from the others was the dominant dealing approach of the students with clinical errors. Disclosing errors of the students was faced with anger of nurses and physicians, patients, their relatives, the instructor, and feeling guilty of the students. "Not asking for help or guidance, lack of knowledge and skill, not enough supervision by senior nurse or instructor" were some of related factors for occurrence of clinical errors by the novice learners. The most frequent error occasion was when a trainee received an oral order. Conclusion: The live experiences of the students indicated that error events are inevitable in clinical areas and their dominant dealing approach is hiding error. It is very important that students learn and understand ethical and legal approaches of managing clinical error before they enter real practice.
