تاثیر دو شیوه تمرین تناوبی سرعتی و ترکیبی (قدرتی-هوازی) بر سطوح آپلین پلاسمایی و مقاومت به انسولین زنان دیابتی نوع2
Effects of two Exercise Modalities of Sprint Interval Training and Combined Training (Strength-Aerobic) on Serum Apelin levels and Insulin Resistance in Women with Type 2 Diabetes

مقدمه: فعالیت ورزشی ابزار درمانی موثری برای مدیریت بیماران دیابتی نوع 2 است؛ هدف این پژوهش بررسی اثر هشت هفته تمرینات ترکیبی و تناوبی سرعتی شدید بر سطوح آپلین پلاسمایی و مقاومت به انسولین زنان دیابتی نوع 2 است.<br>
روش کار­: 52 زن دیابتی نوع 2دارای اضافه وزن (سن:60-45 سال و شاخص توده بدنی>30) با سطوح HbA1c≥ 5/6 % به طور داوطلبانه انتخاب شدند وبر اساس مقادیر هموگلوبین A1c در سه گروه تمرین:SIT (17n=) و ترکیبی (17n=) و کنترل (18n=) قرار گرفتند. تمرینات شامل 12 هفته تمرین ترکیبی و SIT بود. سطوح شاخص­های هموگلوبین A1c و گلوکز ناشتا و انسولین اندازه­گیری شد. جهت تعیین تفاوت­های میان گروهها ازروش آماری تحلیل کوواریانس و در صورت معنی­داری ازآزمون LSD برای تعیین اختلاف بین گروه­ها استفاده گردید. <br>
یافته­ ها: کاهش معنی­داری در میزان گلوکز ناشتا گروه SIT وجود داشت (001/0p<). میزان انسولین سرم گروه SIT و ترکیبی افزایش قابل توجهی یافت (001/0>p و001/0=p). میزان آپلین تغییرات معنی­داری در گروه SIT و ترکیبی وجود نداشت ( 13/0=p و09/0=p). نتایج تفاوت معنی­داری را در شاخص مقاومت به انسولین (HOMA-IR) در گروه (001/0p<) SIT و ترکیبی (001/0p= ) نشان داد. نتایج آزمون آنکوا تفاوت معنی­داری در قندخون ناشتا(171/0 p=و 853/1F=) و آپلین (12/0=p و 511/0=F)نشان نداد. اما تفاوت بین گروهی معنی­داری در انسولین (036/0=p و622/3=F) و مقاومت به انسولین (008/0=p و511/5=F) مشاهده شد.<br>
نتیجه ­گیری: تمرینات ورزشی مستقل از نوع و روش انجام میتوانند اثرات مثبتی بر کنترل قندخون زنان دیابتی نوع 2 داشته باشند.<br>
Introduction: Exercise activity is an effective treatment tool for the management of type 2 diabetic patients. The purpose of this study is to investigate the effects of eight weeks combined and sprint interval training (SIT) on serum apelin levels and insulin resistance in women with type 2 diabetes.<br>
Methods: Fifty-two overweight female type 2 diabetic patients (aged 45-60 years old, BMI > 30) with HbA1C value of 6.5% or were assessed for eligibility. Participants were assigned to SIT group (n = 17), combined training group (n = 17), and control group (n = 18) according to HbA1c levels. The exercises included 12 weeks of combined training and SIT. To determine the differences between groups, a statistical analysis of covariance and a meaningful LSD test were used to determine the difference between groups.<br>
Results: Difference was significant for the fasting blood glucose in the SIT groups (p<0.001). Serum insulin levels showed significant increases in the SIT (p<0.001) and Com (p=0.001). Changes in apelin were not significantly different within SIT and combined training groups (p = 0.13 and p = 0.09, respectively). The data showed significant differences in insulin resistance index (HOMA-IR) in SIT (p<0.001) and Com (p=0.008). Furthermore, the ANCOVA test showed that there were no significant differences in fasting blood glucose concentrations (F = 1.853, p = 0.171), apelin (F = 0.511, p = 0.12) But, significant differences were seen between groups in insulin (F = 3.622, p = 0.036), and HOMA-IR (F = 5.511, p = 0.008).<br>
Conclusion: exercise training independent of mode of training is an effective training method to improve glycemic control in woman with type 2 diabetes.<br>
