تعهد سازمانی و عوامل مرتبط با آن در پرستاران تازهکار

مقدمه: حرفه پرستاری با کمبود نیروی انسانی و افزایش تقاضا برای دریافت خدمات درمانی مواجه است. با این حال به سبب شرایط کاری و مشکلات خاص حرفه، تمایل به ترک شغل در بین پرستاران دیده میشود. بنابراین یکی از مهمترین چالشهای سیستم بهداشت و درمان داشتن کارکنان با تعهد قوی نسبت به سازمان است. این پژوهش با هدف تعیین تعهد سازمانی و عوامل مرتبط با آن بین پرستاران تازه کار انجام شده است.<br>
روش کار: این مطالعه یک مطالعه مقطعی از نوع توصیفی ـ تحلیلی بود که از فروردین تا خرداد 1397 انجام شد. نمونههای پژوهش شامل 600 نفر پرستار تازه کار بودند، که به روش نمونه گیری سهمیهای بصورت در دسترس از 26 بیمارستان وابسته به دانشگاه علوم پزشکی گیلان انتخاب شدند. دادهها با استفاده از ابزاری شامل پرسشنامههای مشخصات فردی_شغلی، بی نزاکتی در محل کار (کورتینا، 2001)، رفتارهای زورگویانه در محل کار (اسکارتین 2010)، عاطفه منفی (پرایس، 2001)، عاطفه مثبت (پرایس، 2001)، شانس ارتقاء (پرایس، 2001) و تعهد سازمانی (پرایس، 2001) که توسط پرستاران تازه کار تکمیل شد، جمع آوری گردید. آنالیز دادهها با استفاده از آزمونهای ضریب همبستگی اسپیرمن، من ویتنی، کروسکال والیس و رگرسیون لجستیک انجام شد.<br>
یافتهها: میانگین نمره تعهد سازمانی 66/0 ± 14/3 (در دامنه 5-1) بود. نتایج حاصل از رگرسیون خطی نشان داد که متغیرهای بی نزاکتی در محل کار (004/0 = P، 55/0-89/0 = CI95%، 70/0 = OR)، عاطفه منفی (030/0 = P، 59/0-97/0 = CI95%، 75/0 = OR)، عاطفه مثبت (038/0 = P، 01/1-74/1 = CI95%، 33/1 = OR)، شانس ارتقاء (001/0 > P، 33/2-00/5 = CI95%، 41/3 = OR)، رضایت از دریافتی (001/0 > P، 62/1-31/4 = CI95%، 65/2 = OR) و انتخاب حرفه پرستاری بر اساس علاقه (002/0 = P، 32/1-62/3 = CI95%، 19/2 = OR) میتواند به طور معنی داری تعهد سازمانی را پیش بینی نماید.<br>
نتیجه گیری: بر اساس نتایج این مطالعه، مدیران پرستاری میتوانند با افزایش شانس ارتقاء، بهبود عواطف پرستاران، حذف سوء رفتارها و افزایش حقوق پرستاران، تعهد سازمانی را در پرستاران تازه کار افزایش دهند.<br>
<strong>Introduction:</strong> Nursing profession faces a shortage of human resources due to increasing demand for health services. However hard working and some special problems in nursing increased intention to leave in nursing. Therefore, one of the most important challenges in the healthcare system is having employees with a strong commitment to the organization. This research conducted to determine organizational commitment and related factors among novice nurses.<br>
<strong>Methods:</strong> This was an analytical, descriptive, cross-sectional study conducted on March to May 2018. Research samples consisted of 600 novice nurses who were selected through quota consensus sampling from 26 hospitals affiliated to Guilan University of Medical Sciences. Data were collected using tools including personal work-related characteristics, the workplace incivility (Cortina, 2001), the workplace bullying behaviors (Escartín, 2010), the negative affectivity (Price, 2001), the positive affectivity (Price, 2001), the promotional chance (Price, 2001) and the organizational commitment (Price, 2001) that were completed by novice nurses. Data analysis was carried out using Spearman’s rho, Mann-Whitney, Kruskal-Wallis, and Stepwise (LR) multiple logistic regression.<br>
<strong>Results:</strong> The mean score of organizational commitment was 3.14 ± 0.66 (range: 1-5). The results of linear regression indicated that workplace incivility (OR= 0.70, C.I.95%= 0.55-0.89, P= 0.004), negative affectivity (OR= 0.75, C.I.95%= 0.59-0.97, P= 0.030), positive affectivity (OR = 1.33, C.I.95% = 1.01-1.74, P= 0.038), promotional chance (OR = 3.41, C.I.95% = 2.33-5.00, P˂ 0.001), satisfaction with the salary (OR = 2.65, C.I.95% = 1.62-4.31, P˂ 0.001) and interest in the nursing profession (OR= 2.19, C.I.95%= 1.32-3.62, P= 0.002) could significantly predict nurse’s organizational commitment.<br>
<strong>Conclusions:</strong> According to the results of this study, nursing managers can increase organizational commitment in novice nurses by increasing their chances of promotion, improving nurse’s affections, eliminating mistreatments and increasing nurses' salaries.<br>
