تأثیر استفاده همزمان قفل هوا و طول مدت تزریق برروی شدت درد و میزان کبودی ناشی از تزریق زیرجلدی هپارین

مقدمه: کبودی و درد از شایعترین عوارض ناشی از تزریق زیرجلدی هپارین میباشد که موجب محدود کردن ناحیه تزریق، اضطراب، اختلال در تصویر ذهنی از خود، امتناع از درمان و کاهش اعتماد بیمار نسبت به عملکرد پرستار میشود. هدف از مطالعه حاضر مقایسه تأثیر قفل هوا و طول مدت تزریق بر میزان کبودی و شدت درد ناشی از تزریق هپارین زیرجلدی است.
روش: این مطالعه نیمه تجربی برروی 35 بیمار تحت درمان با هپارین زیرجلدی، بستری در بخشهای ICU، اورژانس و داخلی قلب مرکز آموزشی درمانی امام خمینی تهران انجام شد که به روش نمونهگیری مبتنی بر هدف انتخاب شدند. برای هر بیمار دو تزریق 10 ثانیهای بدون استفاده از قفل هوا (روش معمول) و 30 ثانیهای به همراه استفاده از قفل هوا (روش مورد مطالعه) در ناحیه شکم در سمت راست یا چپ به صورت تصادفی انجام گردید. فاصله دو تزریق 12 ساعت بود و میزان کبودی 24 و 48 ساعت بعد از تزریق با استفاده از خطکش پلاستیکی و شدت درد بلافاصله و 24 ساعت بعد از تزریق با استفاده از معیار VAS اندازهگیری شدند.
یافتهها: اندازه کبودیهای ایجاد شده ناشی از تزریقات به روش مورد مطالعه، به طور معناداری کمتر از اندازه کبودیهای ایجاد شده به دنبال انجام تزریق به روش معمول بود (001/0p<). میانگین اندازه کبودی در روش معمول در 24 و 48 ساعت بعد از تزریق هپارین زیرجلدی به ترتیب 791/1 و 817/1 سانتیمتر و میانگین آن در روش مورد مطالعه به ترتیب 737/0 و 854/0 سانتیمتر بود. شدت درد نیز بلافاصله بعد از تزریق در روش مورد مطالعه کمتر از تزریق به روش معمول بود (004/0p<) اما شدت درد در دو روش معمول و مورد مطالعه 24 ساعت بعد از تزریق اگرچه میانگین درد در روش معمول (8/0) بیشتر از روش مورد مطالعه بود (54/0) اما این تفاوت معنادار نبود (203/0=p).
نتیجهگیری: با توجه به کاهش چشمگیر اندازه کبودی و شدت درد ناشی از تزریق زیرجلدی هپارین به دنبال افزایش مدت زمان تزریق و استفاده از قفل هوا، به منظور ارتقاء کیفیت مراقبتی و به حداقل رساندن تجربیات ناخوشایند و استرسزای بیماران، افزایش طول مدت زمان تزریق به 30 ثانیه و استفاده از قفل هوا در تزریق زیرجلدی هپارین توصیه میشود.
Introduction: Bruising and pain are the most common complications of heparin subcutaneous injection that can be leads to limited areas for next injections, anxiety, disturbance of body image, refusal of treatment by patient and reduce the effectiveness of nurse-patient trust. Although various methods have been proposed to minimize this unwanted effects but so far none of these methods has not been able to effectively reduce the adverse effects. The aim of this study was to compare the effects of air locks and duration of pain and bruising caused by the subcutaneous heparin injection.
Method: This Quasi-experimental study, done on 35 patients that treated with subcutaneous heparin in ICU, CCU, emergency and cardiology wards in Imam Khomeini hospital in Tehran. For each patient, two injections of 10 seconds without the use of an air lock (the usual method), and 30 seconds with the use of air lock (case study method) were carried out in the abdomen on the right or left, randomly. The interval between injections was 12 hours. Bruising 24 and 48 h after injection was measured using a plastic ruler and pain was measured using the VAS scale immediately and 24 hours after injection.
Results: Size of the bruises caused by the injections in study method, significantly less than the size of the bruises were caused by injection in the usual method (P≥0.0). Average size of the bruises in the usual method at 24 and 48 h after injection of subcutaneous heparin were 1.791 and 1.817 respectively, And 0.854 in study method. Pain intensity immediately after injection in study method was significantly less than it in usual method (P≥0.004). Difference in pain intensity in two method 24 hours after injections was not significant, although average of pain intensity in study method was less than pain in usual method.
Conclusion: Due to the dramatic reduction in size of the bruising and pain caused by subcutaneous injection of heparin, looking to increase the injection time and the use of air lock in order to improve the quality of care and minimize unpleasant and stressful experience for patients, increasing the injection duration to 30 seconds and use an air lock heparin subcutaneous injection is recommended.
