اثر تمرینات پیلاتس بر تغییرات تعادل، ترکیب بدنی و فاکتورهای حیاتی (فشارخون دوگانه و ضربان قلب استراحتی) زنان بیمار مبتلا به مولتیپل اسکلروزیس
Effects of Pilates on Changes in Balance, Body Composition, and Vital Signs Including Dual Blood Pressure and Resting Heart Rate in Females With Multiple Sclerosis in Tabriz, Iran

مقدمه: مولتیپل اسکلروزیس (MS) بیماری مزمن پیشرونده سیستم عصبی مرکزی است که با علائم و عوارض ناتوانکننده همراه است و موجب بروز اختلالهای شدید ازجمله تعادل میشود. هدف این مطالعه، بررسی اثر تمرینات پیلاتس بر تغییرات تعادل، ترکیب بدنی و فاکتورهای حیاتی زنان بیمار مبتلا به (MS) است.<br>
روش کار: این مطالعه از نوع نیمه تجربی که پس از تائید در کمیته اخلاق دانشگاه در سال 1395 اجرا شد. جامعه پژوهش حاضر، بیماران زن مبتلا به ام. اس شهر تبریز بود. از بین آنها 34 زن مبتلا به ام.اس به صورت نمونههای در دسترس انتخاب و بهطور تصادفی در دو گروه تمرین و کنترل قرار گرفتند. معیار ورود به مطالعه: بدون تغییرات دارویی در طول دوره مطالعه، امتیاز وضعیت گسترش ناتوانی آنها بین (0 تا 6) باشند، دامنه سنی 40-20 سال باشد و معیارهای خروج از مطالعه نیز عبارت بودند از: عدم سابقه بیماریهای ارتوپدی، قلبی ریوی یا متابولیک و بروز علائم نورولوژی شدید و ناگهانی مانند از دست دادن حافظه یا ضعف در قضاوت، تشنج صرعی، افسردگی شدید، بیماری گیجی، مشکل شدید تعادل یا سرگیجه، از دست دادن شنوایی، بیماری پارکینسون، آلزایمر و ... در 3 هفته قبل از شروع طرح باشد. پروتکل گروه تمرین پیلاتس هشت هفته سه جلسهای 60 دقیقهای بود. 24 ساعت قبل و بعد از دوره تمرین، ارزیابی متغیرهای موردنظر انجام شد. سنجش تعادل با آزمون (TUG)، ترکیب بدن با روش سهنقطهای جکسون پولاک، فشارخون با دستگاه امرون دیجیتالی و ضربان قلب استراحتی بهوسیله تعداد ضربان قلب در یک دقیقه بهروش مچ دستی اندازهگیری شد. دادهها با آزمون تی زوجی و مستقل در سطح معناداری 05/0 ≥ P و نرمافزار 20SPSS تحلیل شد.<br>
یافتهها: تمرین پیلاتس در درصد چربی، شاخص توده بدنی، نسبت دور کمر به لگن، تعادل، فشارخون سیستولی و ضربان قلب استراحتی گروه تجربی تفاوت معناداری نشان داد (05/0 > P). ولی در مقادیر وزن بدن و فشارخون دیاستولی آزمودنیها تفاوت معناداری مشاهده نشد (05/0 ≥ P).<br>
نتیجه گیری: با توجه به اثر معنادار تمرینات پیلاتس بر تعادل، ترکیب بدن و فاکتورهای حیاتی زنان مبتلا به (MS) میتوان این روش تمرینی را بهعنوان راهکاری جدید و غیرتهاجمی برای مقابله با روند توسعه مشکلات جسمی این بیماران پیشنهاد کرد.<br>
Introduction: Multiple Sclerosis (MS) is a chronic progressive disorder with debilitating symptoms, which is associated with severe problems such as imbalance. The present study aimed at investigating the effects of Pilates exercise on changes in balance, body composition, and vital signs in females with MS.<br>
Methods: The current quasi-experimental study was carried out after approval in the Ethics Committee of Shahid Madani University in 2016. The present study was performed on female patients with MS in Tabriz. Of the statistical population, 34 females with MS were selected by the random sampling method and randomly assigned to two groups of exercise and control. The inclusion criteria were lack of a drug change during the study period, disability status score of 0 to 6, and the age range of 40 to 20 years. The exclusion criteria were no history of orthopedic, cardiopulmonary, or metabolic diseases, and severe and sudden neurological symptoms such as memory loss or poor judgment, seizure, severe depression, dizziness, severe balance or dizziness, hearing loss, Parkinson's disease, Alzheimer's disease, etc. three weeks before the project onset. The exercise group underwnt three 60-minute sessions of Pilates exercise for eight weeks. The studied variables were measured 24 hours before starting and 24 hours after the complication of the exercise protocol in both groups. Balance was assessed via Timed Up and Go (TUG) test; body composition was assessed using Jackson-Pollock 3-site skin fold test, blood pressure was measured by the Omron blood pressure monitor, and using a wrist‐worn heart rate monitor. The data were then analyzed through paired samples t test; P ≤ 0.05 was the level of significance. Data were analyzed with SPSS version 20.<br>
Results: Pilates exercise induced significant changes in fat content, body mass index (BMI), waist–hip ratio, balance, systolic blood pressure, and resting heart rate in experimental group (P ˂ 0.05). However, it failed to indiuce significant difference in weight and diastolic blood pressure (P ≤ 0.05).<br>
Conclusions: Considering the significant effect of Pilates exercise on balance, body composition, and vital signs in females with MS, the exercises are suggested as a novel, non-aggressive method to suppress the progression of physical problems in such patients.<br>
