Background : Hypertension is the most common chronic disease. It is considered a major challenge in global public health. The present study was conducted to determine the effect of self-management training program using peer support on adherence in patients with hypertension. methods:This clinical trial study was conducted on 90 hypertensive patients in Kashan city. Patients were randomly divided into control and intervention groups. The intervention group was trained by peers in 5 sessions during 5 weeks, the control group received usual care. Two groups completed the treatment compliance questionnaires before the, immediately after the end of the intervention and 45 days later. The data was analyzed using spss version 16 statistical software and t-test, chi-square, repeated measures ANOVA tests. Results: The results showed that there was no difference in the treatment compliance score before the intervention in the two groups. But immediately after the intervention and 45 days later, there was a statistically significant difference in the total score of adherence to the treatment, and all its dimensions except the smoking dimension in the two groups (p<0.05). Adherence to treatment varied over time, and the interaction between group and time was significant in this variable (p<0.05). Conclusion: The results of the present study showed that although education through peer support was able to improve adherence to treatment in patients with hypertension. Therefore, this method can be used as a simple method in promoting adherence to treatment and self-management in patients with hypertension.
