Predictive Power of the National Early Warning Score in Predicting Admission and Mortality among Respiratory Patients Presenting to Emergency Departments

Introduction: Respiratory diseases are the leading causes of mortality and disability worldwide and in Iran, and patients often present to hospitals as emergencies. Early identification of patients at risk of deterioration is essential to improve outcomes.
Methods: This multicenter cross-sectional study was conducted on the records of 755 patients ≥18 years of age with respiratory problems who presented to selected emergency departments of Tehran University of Medical Sciences between 2020 and 2023. Demographic, clinical, and NEWS scores were extracted. The predictive power of NEWS was evaluated using statistical tests, ROC curves, and Youden index.
Results: The mean age of the patients was 60.6 years, and 57.9% of them were male. The mean NEWS score was reported to be 8.0 (±3.7). NEWS was able to provide a good prediction of ICU admission (AUC=0.761; cutoff point=9; sensitivity=60.9%; specificity=82.2%). This index also had a higher accuracy in predicting in-hospital mortality (AUC=0.878; cutoff point=9; sensitivity=78.0%; specificity=84.6%).
Conclusion: NEWS score is a valid and efficient tool for predicting ICU admission and mortality in respiratory patients in the emergency department and can be used as a simple and rapid tool in clinical decision-making and patient triage. The use of this index in the Iranian health system can help improve the management of high-risk patients.
