Comparing the Effectiveness of Self-Determination Training, Cognitive Behavioral Therapy and Short-Term Psychodynamic Therapy on Symptoms of Social Anxiety Disorder in Transgender Individuals

Introduction: Transgender individuals experience high rates of social anxiety disorder due to social discrimination and stigma. Accordingly, the aim of this study was to compare the effectiveness of Self-Determination Training, Cognitive Behavioral Therapy and Short-Term Dynamic Psychotherapy on social anxiety symptoms in transgender individuals.
Methods: The current study employed a quasi-experimental research design with a pretest-posttest and control group. The statistical population comprised all transgender individuals residing in Tehran during the second half of 2024. Using the snowball sampling method, 60 individuals were selected and randomly assigned to three experimental groups (15 participants each) and one control group (15 participants). The first experimental group received the Steps to Self-Determination package by Field and Hoffman (1994), the second group received Cognitive Behavioral Therapy (Frey, 1999) and the third group underwent Short-Term Dynamic Psychotherapy based on the Davanloo approach (2001). Participants in all four groups completed the Social Anxiety Scale for Transgender and Gender Diverse Individuals (Mussap & Ho, 2020) at the beginning (pretest) and end (posttest) of the study. Data were analyzed using univariate analysis of covariance and the Bonferroni post-hoc test, at a significance level of 0.05, utilizing SPSS version 28 software.
Results: The effectiveness of all three methods; Self-Determination Training, Cognitive Behavioral Therapy and Short-Term Dynamic Psychotherapy in reducing social anxiety symptoms in transgender individuals was confirmed (Eta=0.509, p<0.001). However, Cognitive Behavioral Therapy demonstrated a significantly greater effect in reducing social anxiety in transgender individuals compared to both Self-Determination Training and Short-Term Dynamic Psychotherapy (p<0.001). Furthermore, no difference was observed between Short-Term Dynamic Psychotherapy and Self-Determination Training regarding their impact on social anxiety in this population (p>0.05).
Conclusion: Overall, it can be concluded that the first line of treatment for social anxiety disorder in transgender individuals is cognitive behavioral therapy. However, self-determination training and short-term dynamic psychotherapy will be useful as complementary treatments.
