Comparison of the Effectiveness of Schema Therapy and Acceptance and Commitment Therapy on Psychological Well-Being in Married Women with Type 2 Diabetes

introduction: Women with diabetes are exposed to various physical and social problems that may adversely affect their mental health. The present study aimed to compare the effectiveness of schema therapy and acceptance and commitment therapy on the psychological well-being of women with type 2 diabetes.
Methods: This quasi-experimental study employed a pretest-posttest design with a control group and a follow-up assessment. The statistical population comprised all women with type 2 diabetes in Tehran in 2025. Among them, 45 women who had referred to medical clinics were selected through convenience sampling and randomly assigned to three groups: a schema therapy group, an acceptance and commitment therapy group, and a control group. Data were collected using the Ryff Psychological Well-Being Questionnaire (1989). The data were analyzed using mixed-design analysis of variance with SPSS version 24.
Results: Comparison of pretest and posttest scores between the experimental and control groups showed that psychological well-being scores increased significantly in both experimental groups (schema therapy and acceptance and commitment therapy), and these improvements were maintained at follow-up. In addition, schema therapy appeared to be slightly more effective than acceptance and commitment therapy in improving some dimensions of psychological well-being (p ≤ 0.01).
Conclusion: The findings indicate that both schema therapy and acceptance and commitment therapy are effective in improving the psychological well-being of women with type 2 diabetes. Therefore, mental health professionals, particularly psychologists working in medical clinics or hospitals, are recommended to utilize these two therapeutic approaches, especially schema therapy, to improve the mental health of this population. Health policymakers are also encouraged to facilitate access to mental health services for this group.
