Introduction: Patients undergoing hemodialysis treatment face different complications due to various drug treatments and various problems. One of these side effects is stress, depression and anxiety disorders, and one of the ways to reduce these side effects in hemodialysis patients is to increase the self-care of the patients. Various models have been introduced to increase self-care of patients, and one of these models is the five-A model..Therefore, the present study was conducted with the aim of investigating the effect of a self-management program based on the Panj-A nursing model on the level of stress, depression and anxiety of patients undergoing hemodialysis in Mahmoudabad Martyrs Hospital, Mazandaran in 2024.
Methods: The present study is a semi-experimental study of two groups, before and after, which was conducted on 30 available hemodialysis patients over 18 years of age who referred to the hemodialysis department of Mahmoudabad Martyrs Hospital in Mazandaran in 1403, who were divided into two groups based on the block division of even and odd days (taking into account the holidays on Fridays). To conduct this study, the patients in the intervention group underwent face-to-face training in the waiting room of the dialysis ward for twelve weeks during five stages according to the study objectives and based on the five-A model. The data collection tool included two parts of the demographic characteristics questionnaire and the DASS-21 questionnaire. Descriptive statistics and independent t, paired t, chi-square and ANCOVA statistical tests were used for data analysis using S.P.S.S software version 20.
Results: The findings of the present study showed that before training in the intervention group, the average score of stress (14.00 ± 2.39), anxiety (12.66 ± 2.58) and depression (15.06 ± 2.60) and in the control group the average score of stress (14.53 ± 1.76), anxiety (11.33 ± 2.22) and depression (16.53 ± 2.32) which there was no statistical significance (p<0.001), but after completing the training, the average score of stress (9.86 ± 1.59), anxiety (9.06 ± 1.66) and depression (10.00 ± 1.51) in the intervention group and in the control group the average score of stress (13.56 ± 1.21), anxiety (12.00 ± 2.13) and depression (16.26 ± 2.91) which the differences was statistically significant (p<0.001).
Conclusion: The results of this study showed that the implementation of a self-care program based on the 5A model can reduce the stress, anxiety and depression scores of hemodialysis patients. Therefore, it seems that the use of the 5A model in the health system as an effective, quick, accessible and easy intervention can be effective in the health of patients, especially the elderly.
