The Effect of Model-Based Self-Management Program 5a on Quality of Life of Elderly Patients with Diabetes

Introduction: Improving the quality of life in diabetes patients is the main objective of health improvement programs. Implementing self-management programs is one of the effective strategies for quality of life and behavior change. Then it can be used of appropriate application models in elderly patients with diabetes to improve efficacy and the proper behavior change. The aim of this study was to evaluate the effect of self-management 5A on the quality of life of elderly patients with diabetes.
Methods: This clinical trial, two groups that 90 elderly patients with diabetes patients admitted in the diabetic clinic Razi Hospital in Ghemshaher city, 2019. The elderly patients were selected by convenience sampling method and were randomly divided into experimental group (45 patients) and control (45), respectively. The experimental group self-management programs was based 5A. The control group received routine hospital. The instrument used in this study is lipad quality of life questionnaire. In the first and 12 weeks after the study was completed in two groups. Data using SPSS 21 software and were analyzed with Chi-square, U Mann Whitney, and Wilcoxon tests.
Results: The findings showed that the most of patients were female, married and elementary. The mean age of patients was 67.48±6.77 years. The mean scores in all aspects of quality of life was higher in the group experimental after completion of intervention. Statistically significant difference was not observed (P<0.001).
Conclusions: This study demonstrated that self-management program 5A model on quality of life of elderly patients with diabetic in the study period has effectiveness. Thus, it is concluded that the diabetic individuals can be significantly improved following instruction by nurses.
